The association of Family Health Teams of Ontario (AFHTO) recognized three Family Health Teams of the South East Local Health Integration Network with four awards at its conference in Toronto.
The “Bright Light” Award, which recognizes leadership, outstanding work and significant progress being made to improve the value delivered by Family Health Teams across Ontario, was awarded to Prince Edward, Queens and Central Hastings family health teams.
The Prince Edward received the Integration: Building Beyond the Family Health Team Award for its implementation of the Prince Edward Ambulatory Cardiac Health (PEACH) Cardiac Rehab Program.
Queens was honoured with an Innovation in Interprofessional Collaborative Team Implementation Award for designing collaborative processes to benefit patients and the Meaningful Use of Electronic Medical Records Award for development of a new form to better collect and retrieve information.
The Central Hastings Using Data to Improve Care Award honours its Diabetes Mellitus (DM) Focus Group.
“These successes demonstrate how the FHTs of the South East LHIN are working to improve patient care by using enabling technologies and planning collaborative care,” said Paul Huras, CEO, South East LHIN. “We could not be happier that the AFHTO has recognized the achievements of the South East LHIN’s care providers in working better and together.”
